Description

phpMyFAQ 3.2.0-alpha before 4.1.9 and 4.2.0-alpha before 4.2.0-beta contains an authentication bypass vulnerability that allows IdP-authenticated users to skip TOTP two-factor authentication through the Keycloak and Entra ID SSO callbacks. Attackers holding a valid or phished IdP identity linked to a 2FA-enabled account can complete the OIDC flow and obtain a fully authenticated session, including administrator accounts.

Executive Summary

This vulnerability in phpMyFAQ versions 3.2.0-alpha to 4.1.8 and 4.2.0-alpha to 4.2.0-beta allows attackers to bypass TOTP two-factor authentication (2FA) during SSO login via Keycloak or Microsoft Entra ID. The flaw occurs because SSO callback controllers grant full authenticated access immediately after IdP validation without checking if the linked account has 2FA enabled, effectively reducing security to single-factor authentication.

Detection Guidance

To detect this vulnerability, check if your phpMyFAQ instance uses Keycloak or Entra ID SSO and if 2FA is enabled for accounts. Inspect the SSO callback controllers (KeycloakAuthenticationController.php, AzureAuthenticationController.php, or services/azure/callback.php) for improper authentication logic that skips 2FA checks.

Impact Analysis

Attackers with valid or phished IdP credentials linked to a 2FA-enabled phpMyFAQ account can gain full access, including administrative privileges, without completing the TOTP verification step. This allows unauthorized access to sensitive data and system control, bypassing critical security controls.

Compliance Impact

This vulnerability may violate compliance requirements that mandate strong authentication (e.g., GDPR's security principle, HIPAA's access controls). Bypassing 2FA undermines security controls required for protecting sensitive data, potentially leading to non-compliance and increased risk of data breaches.

Mitigation Strategies

Disable SSO login temporarily or apply the pending fix from the main branch commits (ebca8fd1, e57e9a03) that enforces 2FA checks before granting sessions. Ensure all SSO accounts, including administrators, require TOTP verification.
