Description

OpenEMR before 8.4.0 contains a mass assignment vulnerability in the patient portal OnsiteDocument query API that lets request parameters set bitwise criteria filters whose values are placed unquoted into the SQL WHERE clause. Authenticated patient portal users can inject SQL via parameters such as Id_BitwiseAnd to read any database table, including other patients' records and staff password hashes.

CWE ID Description
CWE-89 The product constructs all or part of an SQL command using externally-influenced input from an upstream component, but it does not neutralize or incorrectly neutralizes special elements that could modify the intended SQL command when it is sent to a downstream component. Without sufficient removal or quoting of SQL syntax in user-controllable inputs, the generated SQL query can cause those inputs to be interpreted as SQL instead of ordinary user data.

Executive Summary

This is a SQL injection vulnerability in OpenEMR versions before 8.4.0. Authenticated patient portal users can exploit mass assignment to inject SQL via parameters like Id_BitwiseAnd. The values are placed unquoted into the SQL WHERE clause, allowing attackers to read any database table including other patients' records and staff password hashes.

Detection Guidance

To detect this vulnerability, monitor for unusual SQL error messages in OpenEMR patient portal responses, especially those containing database errors. Check for requests with parameters like Id_BitwiseAnd or other bitwise filter parameters being passed unquoted to the SQL WHERE clause. Review logs for error-based SQL injection attempts returning database errors.

Impact Analysis

An attacker with patient portal access could steal sensitive data like other patients' medical records or staff password hashes. If the database user has FILE privileges, they might even execute code. The vulnerability requires only a low-privileged patient session to exploit.

Compliance Impact

This vulnerability likely violates GDPR and HIPAA due to unauthorized access to sensitive patient data. GDPR requires protecting personal data while HIPAA mandates safeguarding protected health information. A breach could lead to legal penalties and reputational damage.

Mitigation Strategies

Immediately upgrade OpenEMR to version 8.4.0 or later to patch the vulnerability. If upgrading is not immediately possible, restrict access to the patient portal API endpoints and disable the OnsiteDocument query API if not required. Implement strict input validation for all request parameters and ensure proper parameterized queries are used to prevent SQL injection.
