Description

OpenEMR before 8.3.0 contains missing authorization and insecure direct object reference vulnerabilities in the oe-module-prior-authorizations module that allow authenticated low-privilege users to access and modify other patients' records. Attackers can request public/reports/list_report.php to view all patients' names, MRNs, insurance and prior-auth data, or submit arbitrary id values to public/index.php to overwrite other patients' prior-authorization records.

Executive Summary

CVE-2026-124456 is a vulnerability in OpenEMR versions before 8.3.0 affecting the Prior-Authorizations module. It involves two flaws: missing authorization checks allowing unauthorized access to patient data and an insecure direct object reference (IDOR) enabling modification of other patients' records. Attackers can view sensitive information like names, MRNs, insurance details, and prior-authorization data or overwrite records by manipulating IDs.

Detection Guidance

Check OpenEMR versions below 8.3.0 by running: grep -r 'openemr' /var/www/html/openemr/version.php or check the version in the OpenEMR web interface. Inspect network traffic for requests to public/reports/list_report.php or public/index.php with manipulated ID parameters.

Impact Analysis

This vulnerability allows authenticated low-privilege users to access and modify other patients' records. Attackers can view or alter sensitive health information, including prior-authorization details, insurance data, and medical record numbers. This could lead to privacy breaches, incorrect medical records, or fraudulent activities.

Compliance Impact

This vulnerability likely violates compliance with GDPR and HIPAA due to unauthorized access and modification of protected health information (PHI). GDPR requires strict data protection and user consent, while HIPAA mandates safeguards for patient data. The exposure and alteration of PHI without proper authorization can result in legal penalties and reputational damage.

Mitigation Strategies

Upgrade OpenEMR to version 8.3.0 or later immediately. If upgrading is not possible, restrict access to the Prior-Authorizations module files (public/reports/list_report.php and public/index.php) via web server rules or disable the module until patched.
