Description

OpenEMR before 8.3.0 contains an insecure direct object reference vulnerability in many encounter-form view and save endpoints that lack patient/encounter ownership checks. Authenticated clinical users can enumerate sequential form id values in requests such as soap/view.php and save.php to read and overwrite other patients' clinical records.

Executive Summary

CVE-2026-124459 is an Insecure Direct Object Reference (IDOR) vulnerability in OpenEMR versions before 8.3.0. It allows authenticated clinical users to access and modify clinical forms belonging to other patients by manipulating form IDs in requests. The flaw exists due to missing patient or encounter ownership checks in multiple form viewer and save handler endpoints.

Detection Guidance

Monitor for unusual access patterns to soap/view.php and save.php endpoints with sequential form IDs. Check logs for requests from authenticated clinical users accessing forms not linked to their active patient or encounter. Use network traffic analysis to detect unauthorized read/write operations on clinical records.

Impact Analysis

This vulnerability can lead to unauthorized access to sensitive protected health information (PHI) such as notes, exams, mental health scores, and billing data. Attackers can also tamper with or delete records across all patients. The impact includes breaches of confidentiality and integrity of patient data.

Compliance Impact

This vulnerability likely violates compliance with GDPR and HIPAA due to unauthorized access and potential modification of protected health information. It compromises data confidentiality and integrity, which are core requirements under these regulations.

Mitigation Strategies

Upgrade OpenEMR to version 8.3.0 or later immediately. Implement patient/encounter ownership checks in all form viewer and save handler endpoints. Review logs for signs of exploitation and restrict access to sensitive endpoints until patches are applied.
