Executive Summary

This is a broken access control vulnerability in OpenEMR versions before 8.3.0. It exists in the portal_payment.php file and allows low-privilege authenticated users to bypass billing access control lists by tampering with patient identifiers (pid and form_pid parameters). Attackers can modify these parameters to view payment pages and record fraudulent payments for arbitrary patients, corrupting billing records.

Detection Guidance

Check OpenEMR versions prior to 8.3.0 by running: grep -r "openemr" /var/www/html/openemr/version.php or check the version in the OpenEMR admin panel. Inspect network traffic for tampered pid or form_pid parameters in portal_payment.php requests. Look for unauthorized payment entries in billing records or logs showing cross-patient financial modifications.

Impact Analysis

An attacker could exploit this to create unauthorized payment entries, alter patient balances, or tamper with billing records. This could lead to financial fraud, incorrect billing for patients, and corruption of healthcare billing data. The integrity of financial records in healthcare environments is at risk.

Compliance Impact

This vulnerability poses significant compliance risks under healthcare regulations like HIPAA. It allows unauthorized modification of billing records, which could violate integrity and audit requirements. GDPR may also be impacted if patient financial data is exposed or mishandled. Compliance violations could result in legal penalties and reputational damage.

Mitigation Strategies

Upgrade OpenEMR to version 8.3.0 or later immediately. Apply ACL checks to enforce billing permissions and remove patient identifier manipulation in portal_payment.php. Verify CSRF tokens are enforced in payment workflows. Audit billing records for unauthorized entries and restrict low-privilege user access to payment functions.
